Hysterectomy is the procedure of removing the uterus from the woman’s body. The uterus is a critical organ of the human reproductive system, responsible for the gestation of the child during pregnancy. The lining within the uterus, known as uterine lining, is responsible for the blood during a menstrual cycle.

However, the uterus is also susceptible to several conditions – some affecting it directly, while others indirectly. In many such scenarios, hysterectomy is considered the last solution to the issue. In a hysterectomy, full or part of the uterus is removed, along with (optional) cervix, ovaries, and fallopian tube. Hysterectomy is an irreversible surgery, after which a woman cannot conceive a child.

  • In case of benign tumors called fibroids growing in the uterus.

Due to the severity of the procedure, hysterectomy has been divided into various degrees. These are:

  1. Partial/supracervical/subtotal hysterectomy removes a portion (usually upper) of the uterus, leaving the cervix intact. This is used in conditions that affect the uterus but not the cervix.
  2. Total hysterectomy removes the entire uterus along with the cervix. This is the procedure commonly understood as ‘hysterectomy.’
  3. In the most severe cases, a total hysterectomy might be combined with Salpingo-Oophorectomy. Known as radical hysterectomy, it removes the uterus, cervix along with the fallopian tubes and ovaries. This is usually done in advanced-stage gynaecological cancers.

Hysterectomy procedure could be broadly divided into two categories- open and MIP. Open surgeries are the traditional surgeries where a women’s body is surgically cut open to perform the hysterectomy. Abdominal hysterectomy is the most common type of open surgery. The other type is MIP or Minimally Invasive Surgery. MIP is preferred much more than open surgeries as they are less risky, less painful, and have a much shorter recovery period. Vaginal hysterectomy is a type of MIP where the incision is made in the vagina, and hysterectomy is performed via that opening. However, the most common and safest type of hysterectomy is laparoscopic.

Laparoscopic Hysterotomy Surgery Delhi procedure

Laparoscopic hysterectomy is a type of MIP involving a laparoscope – a long, thin tube equipped with a high-resolution camera and high-intensity light. The general process is:

  1. Instead of a major cut in the abdomen (as in the case of open surgery), small incisions are made. These incisions are generally multiple, though a single incision might be sufficient in case of a single-sight laparoscopic hysterectomy.
  2. The laparoscope is inserted into the body via the incisions. A monitor allows surgeons to view the insides of your body.
  3. Surgical tools are inserted via the incisions, which start cutting the uterus into small pieces. The pieces are removed one-by-one through the same incisions.
  4. Once the entire uterus is removed, the laparoscope is taken out, and the incisions are closed.

Since hysterectomy involves the removal of a major organ from your body, there are always some risks involved:

  • Urinary incontinence (inability to hold your bladder)
  • Vaginal fistula (an abnormal opening that could connect the vagina to another organ, like bladder or rectum)
  • Vaginal prolapse (a portion of the vagina coming out of the body)
  • Risk of damage to surrounding organs like bladder and intestines.
  • Chronic pain after the surgery, especially in the abdomen.
  • Apart from these, the usual risks associated with the use of anesthetic are also present.

The recovery period of hysterectomy is usually six weeks. During this time, the doctor might recommend you to drink plenty of water, take plenty of rest, refrain from lifting heavy objects, and abstain from sex. The surgery is said to be successful if:

  1. The cause behind the surgery (like vaginal bleeding, cancer, etc.) no longer exists.
  2. There are no persistent side-effects from the surgery.

Life after hysterectomy changes significantly for women, though a lot depends on the type of surgery you had. Women could expect the following:

  • You would no longer experience menstrual cycles. Even if your ovaries were not removed, you would reach menopause sooner than usual. If you were reaching menopausal age before the surgery, your menopause could start immediately after the surgery.
  • A partial hysterectomy will prevent any uterine cancer, but cancer of cervical and ovarian cancer would still exist.
  • You will not be able to become pregnant as you no longer have an essential organ of your reproductive system.

In the case of a laparoscopic hysterectomy, the recovery time is a lot faster. The small wounds heal quickly, and you can feel the relief from your previous condition in a matter of weeks. Since there is a lower chance of side-effects, it also increases its chances of being ‘successful’.
